Online Birth Certificate Application
The Office of the Registrar General guarantees that you will receive your birth certificate within 15 days of completing the online application. NOTE: Proof-of-identity (i.e. Birth Certificate) is required in order to apply for a Social Insurance Number, which is required to legally work in Canada.

Download the PDF form and complete the application directly in the PDF window and then print it. You can submit the printed document to Thunder Bay Service Canada Center at 975 Alloy Drive.
