 -Provide assessment and treatment for patients referred by their health care provider or self-referred
-Provide social work intervention as part of an interdisciplinary team and support programs accross the Family Health Team, such as memory clinic or chronic disease programs
-Assist patients and their families in navigating the health care and/or social service system, which may include case management responsiblities
-Provide groups and/or educational sessions in response to specific needs identified by the Family Health Team
-Participate in health promotion activities such as walking groups and health fairs
-Maintain clear, concise, and accurate documention including use of electonic medical records
-Actively participate in ongoing program evaluation and quality improvement
-Provide education to staff and community about mental health and mental illness
-Participate in commitee work within the NSFHT and with partnering agencies when needed
-Advocate on behalf of a client or group of clients as needed
-37.5 hours/week with the availability to work two evenings a week till 7:00pm for clinic and then as needed for groups and educational sessions
Education and Professional Memberships:
-Master's of Social Work; BSW with experience will be considered
-Current registration with the Ontario College of Social Workers and Social Service Workers
-Must be in good standing with regulatory body
-Valid professional liability insurance (we can guide you in the process if you currently do not have liability insurance)

Skills/ Abilities/ Knowledge/ Experience:
-Experience in a community based or clinical health service setting
-Excellent clinical skills and experience with assessment, treatment and management of mental health problems commonly seen in primary care
-Experience facilitating group programs
-Case management/ System navigation skills and a willingness to research community resources
-Ability to function effectively and efficiently both independantly and as a member of a team
-Excellent verbal and written communication skills
-Excellent organization and time management skills
-Knowledge of electronic medical records (EMR) an asset
