POSITION SUMMARY: The Ontario Native Womens Association is currently accepting applications for a full time position of Youth in Transition Worker in Sioux Lookout. The goal of the Youth in Transition Worker (YITW) is to support youth in their successful transition out of the child welfare system into adulthood. YITWs will help youth to develop and pursue their goals, and will support them to identify, access, and navigate adult service systems relevant to their specific needs. The YITWs will also support youth to connect to existing supports and resources within their communities including, housing supports, education resources, employment services and training, life skills training (e.g. financial management, household management), health and mental health services, and legal services.

A minimum college diploma in Social Services with a minimum of 3-5 years experience in social services. 
Excellent organizational and communication skills and an ability to follow through on assigned duties independently with strict deadlines. 
Knowledge of Not-for-Profit organizations.
Ability to plan, organize and manage time effectively.
Highly motivated individual with the ability to work with minimal supervision. 
Capacity to solve problems and to see different perspectives. 
Knowledge of and connection to service agencies in the location of service delivery (in this case, Sioux Lookout, Ontario)
Ability to network with other community agencies, services and organizations. 
Working knowledge of computers, specifically Microsoft Office..  Working knowledge of standard office equipment, such as photocopier, fax machine and telephone.
Demonstrated experience providing services to Indigenous families and communities. 
Knowledge of Indigenous culture and traditions. 
Ability to speak an Indigenous language is considered an asset.
Valid G-class drivers license and access to an appropriately insured vehicle
Current First Aid/CPR Certification.
Current, valid vulnerable sector criminal records check. 
Ability to travel
YITWs will meet with youth through person-to-person, voice-to-voice, email, social media and other interactions, which may occur in the community or neighbourhood locations.
YITWs will work with local community agencies to both inform them of the YITW program, and become aware of the services available in the community to refer clients. Specifically, YITWs will need to work closely with their local Childrens Aid Services (CAS) to ensure that youth leaving care know about the YITW program, and also to establish a referral process.
YITW will work with youth between the ages of 16 and 24 (inclusive) who meet the following criteria: were subject to a Crown wardship order, legal custody order under section 65.2 of the CFSA, or formal customary care agreement immediately prior to the youths 18th birthday; or were eligible for support under the Renewed Youth Support Policy (RYS) at ages 16 and/or 17, whether or not the youth actually received RYS.
YITW worker will incorporate the traditional Indigenous culture into all programming and services provided to the youth. RESPONSIBILITIES 
Ensure that program eligibility criteria are met. 
Assist eligible youth develop and achieve the goals they have set for themselves through the program.
Support eligible youth leaving care to identify, access and navigate adult service systems relevant to their needs, and transition to adulthood. 
Facilitate access for youth leaving care to existing supports and resources in the community, by providing information and referrals for individual youth that address individual needs and support/reinforce strengths.
Some of the resources and supports may include: 
Housing supports; Education resources; Employment services and training;  Life Skills training (e.g. financial management, household management); Health and mental health services; and Legal services.  Support eligible youth leaving care to develop and maintain relationships with responsible, caring adults in the community to help them as they transition to adulthood.  Foster communication and linkages with community agencies/ organizations to improve access and mitigate barriers to services for youth leaving care.  Work with local CASs to develop a referral approach, and to create awareness of the program among youth leaving care.
Facilitate workshop on healthy living, self esteem, cultural identity, and other related topics 
Services will be: Reflective of and responsive to individual needs. Culturally appropriate. Services will be: Reflective of and responsive to individual needs. Culturally appropriate.  Accountable to the individual and community.  Sensitive to the social, linguistic and cultural diversity of youth. Staffed by individuals with the appropriate range of skills and abilities necessary to respond effectively to the needs of youth. Based on the individual's assessed needs and preferences, and available individual, agency, community and contracted Ministry resources. Good conduct and standard or professionalism at work in representation of ONWA. Adhere to Policies and Procedures as set by the ONWA. To network and promote ONWA and all ONWA related programs. Adhere to professionalism at all times and maintain good communications  Other duties as assigned.
DUTIES ADMINISTRATIVE: The completion of all reporting as stated in the ONWA policies and procedures as well as within the contribution agreement.
Complete all necessary forms for reporting and accountability purposes.
Meet with the Program Manager of Services and provide reports as required.
 STANDARDS OF PERFORMANCE: Adhere to Policies and Procedures as set by the ONWA. AUTHORITY: To execute duties and responsibilities outlined in this job description. ACCOUNTABILITY: The Youth in Transition Worker will be directly accountable to the Program Manager of Services for the proper completion of the functions outlined in the job description.
