Reporting to the Regional Director (Ottawa), the Pharmacy Liaison plays an important role assisting in the day to day operations of the pharmacy and coordinating with the Long-Term Care (LTC) Homes and Retirement Communities we serve. This position offers 1 day per week (to be determined) employment on an on-going basis and involves regular weekly travel within the prescribed Thunder Bay region. Occasional travel within the Greater Toronto Area and/or Ottawa regions may be required for training and meeting purposes.

About Classic Care (a division of Centric Health) Classic Care Pharmacy was founded in 1996 for the purpose of exclusively providing medication and consulting services to Long Term Care Homes and Retirement Residences. With locations in London, Burlington and Ottawa, Classic Care currently service many Municipal, Non-Profit and For-Profit homes throughout Ontario. Classic Care's dynamic team is comprised of Certified Geriatric Consultant Pharmacists, Certified Diabetes Educators, Nurses (RNs, RPNs), Technology Coordinators, Certified Pharmacy Technicians, Pharmacy Assistants, and Customer Care Representatives. They are all dedicated professionals specializing in geriatric care with their primary focus on meeting the unique needs of the residents.

Your Day to Day within the Thunder Bay Region:
- Perform medication audits on-site in Long Term Care (LTC) Homes and Retirement Communities
- Complete/collaborate in the development of medication audit follow-up improvement plans when applicable
- Provide on-site education sessions to staff in the LTC Homes and Retirement Communities on various health, practice, and medication system topics, as applicable (e.g. policy and procedure, web portal, eMAR, influenza protocol, UCP training, nursing practice, etc.)
- Work collaboratively with other team members including the Clinical Nurse, Consultant Pharmacist and Pharmacy Manager in activities supporting applicable homes and communities
- Participate in pharmacy-related transition processes (e.g., new client transitions, eMAR transitions, etc.)
- Attend and actively participate in clinical team meetings
- Work within the Registered Practical Nurse scope of practice as per the College of Nurses of Ontario
- Regular Travel within the region to complete the duties of this position
What you bring to the team:
- Current Registered Practical Nurse Registration in good standing with the College of Nurses of Ontario required
- Bilingual in verbal and written English/French an asset
- Regular/weekly travel to LTC Homes and Retirement Communities
- Demonstrated high level of fluency with written and spoken English
- Working Knowledge of Microsoft Office programs
- Must have a complete understanding of provincial regulations governing LTC homes and Retirement Communities
- Must be familiar with the College of Nurses of Ontario (CNO) Practice Guideline Working with Unregulated Care Providers (UCPs)
- Excellent customer service and interpersonal skills, oral and written communication skills and be able to interact with persons of varying levels of professional expertise in a professional and mature manner
- Highly organized and possess excellent time management skills
- Ability to be flexible regarding scheduling as required
- Skilled in critical thought processes and be able to prioritize appropriately
- Must demonstrate initiative and creativity
- Self-directed requiring minimal supervision and able to work independently
- Motivated to achieve results that reflect a high standard of quality
- Dependable and responsible in the execution of job duties
- Adaptable to internal work changes and environments
- Strong presentation and teaching skills
- Knowledge regarding provincial standards of practice established for LTC pharmacies preferred
- Must possess a Current and Valid Driver's License (province of Ontario) and must have access to a reliable vehicle (mileage/travel expenses reimbursed)
