The Youth in Transition Worker (YITW) is responsible to support youth in their successful transition out of the child welfare system into adulthood.

YITWs will help youth to develop and pursue their goals, and will support them to identify, access, and navigate adult service systems relevant to their specific needs.  The YITWs will also support youth to connect to existing supports and resources within their communities including, housing supports, education resources, employment services and training, life skills training, health and mental health services, and legal services.



  • Meet with youth through person-to-person, voice-to-voice, email, social media and other interactions, which may occur in the community or neighborhood locations.
  • Work with local community agencies to both inform them of the YITW program, and become aware of the services available in the community to refer Community Members. Specifically, YITWs will need to work closely with their local Children’s Aid Services (CAS) to ensure that youth leaving care know about the YITW program, and also to establish a referral process.
  • Work with youth between the ages of 16 and 24 (inclusive) who meet the following criteria: were subject to a Crown wardship order, legal custody order under section 65.2 of the CFSA, or formal customary care agreement immediately prior to the youth’s 18th birthday; or were eligible for support under the Renewed Youth Support Policy (RYS) at ages 16 and/or 17, whether or not the youth actually received RYS.
  • Incorporate the traditional Indigenous culture into all programming and services provided to the youth.
  • Ensure that program eligibility criteria are met.
  • Assist eligible youth develop and achieve the goals they have set for themselves through the program.
  • Support eligible youth leaving care to identify, access and navigate adult service systems relevant to their needs, and transition to adulthood.
  • Facilitate access for youth leaving care to existing supports and resources in the community, by providing information and referrals for individual youth that address individual needs and support/reinforce strengths.  Some of the resources and supports may include:
    • Housing supports;
    • Education resources;
    • Employment services and training;
    • Life Skills training (e.g. financial management, household management);
    • Health and mental health services; and
    • Legal services.
  • Support eligible youth leaving care to develop and maintain relationships with responsible, caring adults in the community to help them as they transition to adulthood.
  • Foster communication and linkages with community agencies/ organizations to improve access and mitigate barriers to services for youth leaving care.
  • Work with local CASs to develop a referral approach, and to create awareness of the program among youth leaving care.
  • Facilitate workshop on healthy living, self esteem, cultural identity, and other related topics.
  • Services will be:
    • Reflective of and responsive to individual needs.
    • Culturally appropriate.
    • Accountable to the individual and community.
    • Sensitive to the social, linguistic and cultural diversity of youth.
    • Staffed by individuals with the appropriate range of skills and abilities necessary to respond effectively to the needs of youth.
    • Based on the individual's assessed needs and preferences, and available individual, agency, community and contracted Ministry resources.

  • Inter-cultural Competency Informed – Understands to incorporate a personal responsibility to increase sensitivity, awareness and implementation of ONWA’s cultural teachings and organizational practices in both professional conduct and work related deliverables.
  • Indigenous Gender-Based Analysis Informed – Understands to incorporate an intersectional approach to gender, race and discrimination and subsequent intergenerational impacts affecting Indigenous women and their families.
  • Trauma-Based Practice Informed – Understands to incorporate a trauma-informed approach to recognize and respond to all forms of trauma and the necessity of supporting Indigenous women in a culture of tradition and safety in her healing journey.
  • She is Wise Leadership Informed – Understands to incorporate the Four Agreements within daily responsibilities and combined with ONWA’s cultural teachings contribute to the empowerment and leadership of Indigenous women within their communities.



  • A minimum college diploma in Social Services with a minimum of 3-5 years experience in social services.
  • Excellent organizational and communication skills and an ability to follow through on assigned duties independently with strict deadlines.
  • Ability to plan, organize and manage time effectively.
  • Highly motivated individual with the ability to work with minimal supervision.
  • Capacity to solve problems and to see different perspectives.
  • Knowledge of and connection to service agencies in the location of service delivery
  • Ability to network with other community agencies, services and organizations.
  • Knowledge and working of Not-For Profit Organizations.
  • Excellent written and oral communications, with emphasis on keen listening and ability to effectively interpret verbal communications and needs of prospective Community Members.
  • Must be dependable, able to follow instructions, respond to management direction and be able to improve performance through management feedback.
  • Excellent working knowledge of Microsoft Office; competency working with other databases.
  • Experience working with Indigenous Peoples, knowledge of Indigenous cultures and traditions.
  • Knowledge and understanding of current issues facing Indigenous Women and their families.
  • Working knowledge of MS Office Software, internet, and general office equipment.
  • Ability to speak an Indigenous Language is considered an asset.
  • Must be willing and able to work a flexible working from home arrangement if required.  This may include a combination of virtual remote work in your home and in office arrangements.  This will be based on operational needs and will be determined by the ONWA.
  • Valid G Class Driver’s License and access to a reliable vehicle with appropriate insurance.
  • Current, valid vulnerable sector criminal records check. Valid 1st Aid/CPR.
  • A cellular phone and internet access is required within this position
  • Ability/willingness to travel and to work flexible hours.
